Go to jotform.com and sign up. The free plan is enough to explore the platform and test a kit before committing to a paid plan. You do not need a credit card to start.
Bronze for a single workflow. Silver for 3-5 active workflows. Gold for HIPAA or heavy usage. Enterprise if you need SSO, dedicated support, or custom terms. I published a full breakdown at /pricing and a calculator at /tools/jotform-plan-calculator.
Gold or Enterprise required. Go to Settings > HIPAA Compliance, enable it, and sign the BAA. This is not automatic - you must do it manually. Without this step, your forms are not HIPAA compliant even on Gold. Full walkthrough at /hipaa.
Kits live at /solutions. Each one is a pre-built workflow for a specific operation: patient intake, donation processing, event registration, and so on. The page tells you exactly what is included - forms, logic, integrations, and email routing.

Everything is editable. Change field labels, add conditions, swap integrations, adjust email templates. Jotform is drag-and-drop - you do not need a developer for most changes. If you get stuck, I offer ad-hoc support.

Not every integration is safe for protected health information. Google Sheets and Slack are not safe for PHI without additional safeguards. Salesforce, HubSpot, and Stripe can be configured safely. Details in the HIPAA section.
Check Settings > Emails. Make sure the recipient email is correct and the conditional logic is not blocking the notification. Also check your spam folder. Jotform emails sometimes land there on first send.
Check your submission limit (My Forms page shows usage). If you hit the limit, forms stop accepting submissions until the next billing cycle or until you upgrade.
Verify your payment integration is connected and live (not in test/sandbox mode). Check that the product fields are configured and the currency matches your payment gateway settings.
