Yes, on Gold and Enterprise plans. Jotform signs a Business Associate Agreement (BAA) and offers end-to-end encryption, PHI field segmentation, and audit logs. Silver and below do not include HIPAA.
Yes. Having a Gold plan does not automatically make your forms HIPAA compliant. You must enable HIPAA compliance in your account settings and sign the BAA. I walk through exactly how in the HIPAA notes.
Not all of them. Google Sheets, Slack, and standard Zapier are not safe for PHI without additional safeguards. Salesforce, HubSpot, and Stripe can be configured safely. I published a detailed breakdown of each integration's PHI safety.

Probably. Jotform has 150+ native integrations and connects to 8,000+ tools via Zapier, Make, and Power Automate. If you do not see it in the native list, Zapier almost certainly covers it.
